• Provide assistance with physical and emotional support of clients, maximising independence and assisting in empowering the clients to set and achieve personal goals.
• Support clients with achieving their desired outcomes of activities of daily living.
• Provide and assist clients with personal care needs as specified in their care plan and report any concerns/issues as necessary.

• To communicate effectively with clients and be able to pass information relevant to the clients’ needs onto other staff as and when required.
• Communicate any concerns immediately to your manager.
• Complete electronic care notes, including accident and incident reporting and adhere to confidentiality policy and handle information appropriately.

• Participate in regular supervisions and appraisals.
• Participate in regular team meetings.

• Undertake mandatory training as required.
